Governor Jindal and the La. Department of Veterans Affairs Secretary Lane Carson are awarding the La. Veterans’ Honor Medal to veterans throughout the state. The intention is to honor and recognize the veterans of Louisiana and to express appreciation and gratitude for their service.

Veterans wishing to receive this Honor Medal must submit an application form with a copy of their DD-214/discharge papers. Click on the link here to download the application form and mail the form to the address on the bottom of the form. You will be contacted regarding the receipt of your medal.
