The Terrebonne Parish Consolidated Government's Head Start Program is accepting applications for children ages 3 to 5 years from low income families for the 2008-2009 school session . Parents of children with severe disabilities are encouraged to apply. To apply you can visit any of these locations.

To apply, please bring the following documentation with you.

1. Proof of Income (2 check stubs, current food stamp printout, 2007 tax return, child support order, SSI Award Letters, etc.)
2. Child's birth certificate
3. Child's updated immunization record
4. Child's Social Security Card
5. Parent's valid Driver's license or Social Security Card
6. Child's Medicaid Card, if applicable
7. Court order of Guardianship of child, if applicable
8. Proof of residency (utility bill, lease agreement)
9. Child's evaluation or I.E.P. (if applicable)
